CARBON MONOXIDE: THE COLOURLESS, ODOURLESS KILLER

When the phrase “air pollution” pops up anywhere, most people tend to limit it to the harmful gases that are suspended into the air due to various human activities.

Whether you are in an enclosed space or in the open, the air around is prone to poisonous elements such as Carbon monoxide.

Carbon Monoxide (CO) otherwise known as “the silent killer” is a poisonous, colorless, odorless, and tasteless gas. CO poisoning typically occurs from breathing it in at excessive levels. It causes severe damage in the body as it displaces oxygen in the blood and deprives the heart, brain, and other vital organs of oxygen.

CO is produced by devices that burn fuels. Therefore, any fuel-burning appliance or machine in your home is a potential CO source. A commonly used machine in an average Nigerian home is the Generator. Given the epileptic situation of the country’s electricity supply, it is not uncommon to find generators in every household and business place.

However, the awareness on the dangers of inhaling this gas seems not to be enough as news of people getting killed by this toxic air is still being reported in the media. CO poisoning remains a major health issue that is underestimated in Nigeria.

The popular Computer Village in Ikeja, Lagos, an information and Communication Technology hub, is one place many businessmen and traders have to rely on noisy and smoky generators to run their operations.

There is little left to the imagination on how CO poisoning is trivialised as every shop owner has generating sets, whether big or small.
Given the overpopulation of the tech hub, it is not hard to see that the air in that environment is as toxic as the word itself. When the electricity supply is cut, all shop owners power their generators to keep the business running.

“I don’t think most of the shop owners here realise how dangerous the smoke coming out of generators are. Look at that corner, you see how cramped up it is? I can bet you that all the shops there have ‘I better pass my neighbour’ generators. They will never hear. 

I once told a man who faced his generator’s exhaust pipe into his shop that it is bad for him to be inhaling that thing. His response was ‘one thing must kill a man’. 

The funny thing is even if someone here dies of inhaling that smoke, they still wouldn’t believe it. They would still think it is a spiritual attack from a competitor”, a shop owner who identified himself as Tochi revealed.

“The symptoms of CO poisoning commonly include headache, dizziness, weakness, vomiting, chest pain and flu-like symptoms. That is why it is easy to dismiss them as the effect of the gas poisoning because the symptoms I mentioned are also related to other illnesses. 

I think the government needs to do better on sensitising the citizens on the dangers of inhaling carbon monoxide and the kinds of machines that emit them. Since it is colourless and mostly mixes with other gases, it is difficult to tell which is which”, a medical doctor at Ifako Ijaiye General hospital said.

Generators should always be placed at least 20 feet away from apartments, shops or offices. It is also advised that the machines should be serviced regularly to reduce the exhaust fumes being emitted while they are being run.

Description of Duties
The incumbent is expected to support national authorities (National, State, or LGA level), partners and other appropriate stakeholders in the following specific tasks:

• Supporting the planning, implementation, supervision and monitoring of NERICC Community 

• Engagement framework

Support in the planning of all operational and programmatic activities for community engagement at the national and sub-national levels

• Support in conducting baseline and end line assessments to determine project outcomes and impact including change in behaviour, health-seeking behaviours of communities regarding immunisation including qualitative methods.

• Using the results of the baseline assessment, support in developing strategic communication plan to target the populations in the project states.

• Support the communication and social mobilization aspect of Routine Immunization intensification and community engagement implementation in project states

• Support the development and implementation of project Visibility strategies

• Support in developing Guidelines, SOPs and Training materials for personnel on community engagement, interpersonal communication skills

• Support the process of identification and community engagement focal persons at state and LGA levels as well their capacity building activities for the selected focal persons

• Support the compilation of monthly CE data (line list, reconciliation, defaulter etc.) and other project deliverables from the implementing states and generation of monthly feedback

• Facilitate the conduct of monthly review meetings with the stakeholders in the 2 states to ensure adequate feedback about program performance.

• Provide regular supportive supervision targeting at de-bottlenecking to the implementing states

• Support the coordination of WHO community engagement support in 6 Routine Immunization MoU states.

• Supporting states to conduct periodic routine immunization intensification for targeted LGAs or communities based on analysis of prevailing immunity gaps and limitations of coverage of current interventions such as Optimized Integrated Routine Immunization Sessions (OIRIS)

• Conduct high level advocacy to traditional leaders to ensure maximum support to community engagement processes at all levels and provide feedback on the traditional institution on their performance

• Support the identification of best practices and support the process of reward and sanctions of traditional leaders by the emirates

REQUIRED QUALIFICATIONS

EDUCATION:

Essential: Secondary, technical or commercial school certificates in health communication, mass communication, and social sciences.

Desirable: University degree in Health Communication, Mass Communication or Social Sciences.

EXPERIENCE:

Essential: At least 10 years’ experience commensurate with the duties of the posts and its grade in an international, public or private organization of repute

LASSA FEVER: WHAT YOU NEED TO KNOW

Lassa fever is a zoonotic disease, meaning that humans become infected from contact with infected animals. The animal reservoir or host of Lassa virus (LASV) is a rodent of the genus Mastomys, commonly known as the multimammate rat. Mastomys rats infected with LASV do not become ill, but they can shed the virus in their urine and faeces.

SYMPTOMS

The incubation period of Lassa fever ranges from six to 21 days. The onset of the disease, when it is symptomatic, is usually gradual

It starts with fever, then general weakness and malaise. After a few days, headache, sore throat, muscle pain, chest pain, nausea, vomiting, diarrhoea, cough and abdominal pain may follow. In severe cases, there is facial swelling, fluid in the lung cavity, bleeding from the mouth, nose, vagina or gastrointestinal tract and low blood pressure may develop. Shock, seizures, tremor, disorientation and coma may be seen in later stages. Deafness occurs in 25 per cent of patients who survive the disease. In half of these cases, hearing returns partially after one to three months. Transient hair loss and gait disturbance may occur during recovery. Death usually occurs within 14 days of onset in fatal cases. The disease is especially severe late in pregnancy, with maternal death and or foetal loss occurring in more than 80 per cent of cases during the third trimester.


TRANSMISSION

Humans usually become infected with Lassa virus from exposure to urine or faeces of infected Mastomys rats. Lassa virus may also be spread between humans through direct contact with the blood, urine, faeces or other bodily secretions of a person infected with Lassa fever. Person-to-person transmission occurs in both community and healthcare settings where the virus may be spread by contaminated medical equipment such as re-used needles.


WHO IS AT RISK?

Lassa fever occurs in all age groups and both sexes.Persons at greatest risk are those living in rural areas where Mastomys are usually found, especially in communities with poor sanitation or crowded living conditions. Health workers are at risk if caring for Lassa fever patients in the absence of proper barrier nursing and infection prevention and control practices.


DIAGNOSIS

Because the symptoms of Lassa fever are so varied and non-specific, clinical diagnosis is often difficult, especially early in the course of the disease. Lassa fever is difficult to distinguish from other viral haemorrhagic fevers such as Ebola virus disease, as well as other diseases that cause fever, including malaria, shigellosis, typhoid and yellow fever.Definitive diagnosis requires testing that is available only in reference laboratories.


TREATMENT

The antiviral drug, Ribavirin, seems to be an effective treatment for Lassa fever if given early on in the course of clinical illness. There is no evidence to support the role of Ribavirin as post-exposure prophylactic treatment for Lassa fever. There is currently no vaccine that protects against Lassa fever.


PREVENTION / CONTROL

Prevention of Lassa fever relies on promoting good community hygiene to discourage rodents from entering homes. Effective measures include storing grains and other foodstuff in rodent-proof containers, disposing of garbage far from the home, maintaining clean households and keeping cats. 

Family members should always be careful to avoid contact with blood and body fluids while caring for sick persons. In healthcare settings, staff should always apply standard infection prevention and control precautions when caring for patients regardless of their presumed diagnosis. 

These include basic hand hygiene, respiratory hygiene and use of personal protective equipment. Laboratory workers are also at risk. Samples taken from humans and animals for investigation of Lassa virus infection should be handled by trained staff. 

Regular hand washing with disinfectants should always be encouraged.
